1) Client authorizes Blue Sky Black Limo Service to send an invoices to the client and must make a full payment by the due date.

If the client makes a mistake in the requested date or time, they can reschedule. We can accommodate the change if we have availability. However, if we do not have availability and it is the customer’s fault, we are not responsible for the mistake.
3) If service is terminated early, no refund will be issued, as all fees are deemed earned when paid.

Changes:
1) Any changes to the client’s original order and the client’s original specifications shall be subject to additional charges if we do have the availability. Therefore, should the client request an increase in the duration of service, additional charges shall apply. Changes to the pickup location may also be subject to a rate increase. Other changes, such as the number of passengers, destination, etc. will also be subject to additional fees. Where applicable, overtime rates shall be billed based on the listed basic charge. Should the client opt for a decrease in the duration of service, the basic original rates shall still apply. Should the client reduce the number of passengers accompanying them during the trip, the basic original rates shall still apply. Should the client request to be driven to a shorter destination, the basic original rates shall still apply. The client shall be responsible for payment of the originally stipulated charges.
2) The client shall pay the full amount for the reserved number of hours and days even if the client uses the company’s services for less time and days, or even if the client does not use the company’s services at all. The client agrees to pay the minimum number of hours set by the company. The client will pay the higher number of hours set in advance by either the company or the client. If the client cannot use the vehicle for the entire period of the contract, they must still make the payment. This is payment in full. This amount is for the time the vehicle and the company’s services were used.
3) The company’s fare is based on an hourly rate and mileage. Therefore, if the client is unable to use the vehicle & company’s services for the entire agreed period due to traffic delays, the client shall still pay for the originally stipulated hours. If there are traffic delays, the client shall still pay for the originally stipulated hours. The same applies for any other reason. The client shall also pay for the originally stipulated mileage.
4) Any changes was made by airlines with date or time, doesn’t affect your reservation with us. We will provide you service anyway.
Vehicle:
The specifications of the vehicle provided to the client are listed on the reverse of this agreement. The vehicle was thoroughly inspected by the company’s management and chauffeur before commencement of service. The client shall inspect the vehicle before boarding. If any damage(s) exist(s) on the vehicle, the client shall note the damage(s) on the front side of this agreement in the indicated space. If the client does not notice any damage, the indicated space shall remain blank. Should the client have noted any damage(s) in the space indicated, the client affirms that by using the company’s service. The client has judged the damage(s) to be negligible. The client also believes the damage(s) are no impediment to the delivery of services by the company. The client understands that the vehicle will be re-inspected by the company’s chauffeur and management immediately upon return to the base. Should any new damages not shown in the space indicated appear on the vehicle upon re-inspection, the client will be fully responsible. The client will be responsible if new damages appear on the vehicle upon re-inspection. The client will be charged for any repairs. Any significant and excessive cleaning costs incurred by the company will also be charged. The company will decide about unusual wear and tear. The decision about damage to the vehicle interior and body shall rest solely with the company. The company shall make every effort to provide a vehicle that is identical. This includes model, size, color, etc. to the client’s request. However, the company does not guarantee that an identical vehicle will be available for the client’s use. The company shall not be liable if an identical vehicle is not available on the date of service. The company does not guarantee the year, color, passenger capacity, model, type dent-free, & brand of vehicle. The company will, at its discretion, dispatch a vehicle subject to availability on the day and time of service.
DRUGS/SMOKING:
No possession, sale or consumption of any type of narcotics or illegal drugs is permitted. Violations will result in immediate termination of contract and services and forfeiture of all paid deposits and fees for service.
UNSAFE & DANGEROUS ROADS:
FOR THE SAFETY OF THE PASSENGERS, DRIVERS, WE WILL NOT GO ON ANY DANGEROUS ROADS.
SUCH AS: ONE WAY ROADS, WINDING ROADS (ZIG ZAG ROADS), STEEP ROADS, DIRT ROADS, HILLS, CLIFF AREAS, ETC
SMOKING & FOOD:
Smoking or food/eating is not permitted inside any of the vehicles in our fleet at any time.
ALCOHOL POLICY:
No alcoholic beverages will be stocked or permitted in vehicles with passengers under the age of 21 without a legal guardian present. Consumption of alcohol by a minor is never tolerated under any circumstance. Alternative beverages are available upon request for additional cost.
QUOTE & BOOKING:
All vehicles are quoted subject to availability at the time of booking. Rates are subject to change depends on New Years Day, New Years Eve, Busy Days, Holidays, Peak Periods and Long Distance. 100% deposit require at the time of booking.
MULTIPLE STOPS OR DESTINATIONS:
Stops during your service should be prearranged before the service starts. Due to the size of our vehicles, our drivers need to know all stops in advance. These precautions are for the safety of each passenger as well as the vehicle. Unscheduled stops are at the driver’s discretion. All pricing are based on original total mileages, destinations, and stops. Please call the office to get pricing for adding any extra stops or adding more miles after booking your reservations.
MAXIMUM CAPACITY:
Drivers are not authorized to exceed the number of passengers stated on your contract. This violates state law and insurance guidelines.
Client accepts full liability for client, client’s guests, and/or client affiliations. The client is responsible for any damages to the vehicle. The charges for damages are as follows: The starting minimum charge will be $300 for vomiting. It will be $380 for each cigarette burn. It will be $100 for gum/glitter on the carpet. It will be $20 for each broken/missing glass.

Weaver Terms And Conditions Rental With Blue Sky Black Limo Service LLC
CONTRACT
The Customer agrees to pay the invoice _____
for the services to be rendered for the Contract Time.
for each additional hour or part thereof worked by the driver either before or after the Contract Time.
WARRANTY
Vendor warrants that the limousine has undergone regular maintenance and is in good, clean condition. The foregoing warranty does not warrant that the limousine will be free from unforeseen mechanical defects.
ASSUMPTION OF RISK BY CUSTOMER
Vendor does not guarantee the safety or assume any responsibility for any personal articles or items lost, stolen, damaged or left in the limousine. Vendor is not responsible for delays in the limousine’s departure and arrival caused by weather, road conditions, hazards. accidents or other unforseen events, including acts of war.
RULES & SPECIAL CONDITIONS
Not more than vehicle capacity listed in the quote previously send to the client by text message shall be provided transport or shelter in the rented limousine. 15 minutes including in the rental service, after 15 minutes extra time, 1 full four fee will be apply.

Deposits Are Non-Refundable
Tips Policy: Gratuity not included in the final invoice
Standard rate of minimum 10% or more
• Conduct causing, or in driver’s opinion likely to cause, damage to the limousine.
• Conduct interfering with, or in driver’s opinion likely to interfere with, safe operation of the limousine.
• Vendor expressly reserves the right to terminate or cancel service without any refund whatsoever if the driver observes violation of the rules as stated above
(Violating the above information may result in cancellation of the rental service without any refunds.)
MAINTENANCE FEES
• A maintenance charges) of $500 (per vehicle) will be applied on all liquid and/or bodily fluids) that are applied on the rented limousine (ex.
Vomit. etc.) • A $250-$500 maintenance charge(s) (per vehicle) will be applied on any smoke aroma left on the rented limousine (ex. Cigarettes and Marijuana) – Maintenance charges) may vary ($500 or more) on any interior damage(s) applied on the rented limousine.
DAMAGE.
Customer agrees to pay for any and all damage and any cleaning to the limousine which results or is required due to the conduct of any person in the limousine Specifically, such damage includes, but is not limited to, burns, spillage, vomiting, broken glassware, scratches, stains and broken windows, seats, and mirrors.
Customer agrees that the charges for such repairs and cleaning will be assessed to the credit card for the individual or entity who has rented the limousine. In the event that the credit card is not able to be charged, customer agrees to remit payment for damages within seven (7) days of the date of the event.
HOLD HARMLESS.
Customer agrees to hold the driver and Vendor harmless for any and all negligence or gross negligence in the maintenance and operation of the limousine. Customer agrees to indemnify the driver and Vendor for any and all costs and fees incurred in the defense of any claim made against them arising out of and in connection with the rental of the limousine. Hollywood Playnight shall not be held responsible for any damages or injuries incurred during the rental of our limousine service.
LIABILITY WAIVER.
in consideration for receiving permission to participate with Blue Sky Black Limo Service. The Customer HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Blue Sky Black Limo Service LLC, and their officers, agents, servants, or employees (hereinafter referred to as RELEASES) from any and all liability, claims demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me and/or other family members that are using the transportation service, or any of the property belonging to me and/or other family members that are using the transportation service WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASES OR OTHERWISE, while participating in such activity, or while in, on or upon the premises where the activity is being conducted.
The Customer is fully aware of the unusual risks involved and hazards connected with this activity, including but not limited to travel risks and/or road hazards. The Customer hereby elect to VOLUNTARILY PARTICIPATE in said activity with full knowledge that said activity may be hazardous to me and my property. THE CUSTOMER VOLUNTARILY ASSUMES FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH that may be sustained by me, or any loss or damage of property owned by me, as a result of being engaged in such activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASES OR OTHERWISE. The Customer further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS THE RELEASES from any loss, liability, damage or costs, including court costs and attorney fees that they may incur due to my participation in said activity,
WHETHER CAUSED BY NEGLIGENCE OF RELEASES OR OTHERWISE.
It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind the members of my family and spouse, if the customer is alive, and my heirs, assigns and personal representative, if the customer is deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above-named RELEASES. The Customer hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Florida. IN SIGNING THIS RELEASE, THE CUSTOMER ACKNOWLEDGES AND REPRESENTS THAT THEY HAVE READ the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made: The customer is at least eighteen (18) years of age and fully competent; and the customer executes this release for full, adequate and complete consideration fully intending to be bound by same.
BREACH OF CONTRACT AND ATTORNEY FEES
If the customer shall breach this contract, the Vendor shall be entitled to terminate service under this agreement immediately and be entitled to his attorney’s fees, the cost of collection and costs incurred in any lawsuits arising out of or in connection with said breach.
SEVERABILITY
If any provisions of this contract are deemed void or unenforceable, the remaining provisions shall remain in full force and effect.
JURISDICTION
The parties agree that all the terms and conditions stated herein shall be construed under the laws of the State of Florida and any action or proceeding brought in connection with or arising from this contract shall be considered under the jurisdiction of Tampa Florida Court.
